How CCP support works
CCP helps keep your care moving between appointments. The goal is to stay connected, follow the care plan, and make sure your doctor's office remains informed.
1. Your doctor identifies the need
Your doctor or practice decides whether this kind of support fits your situation. For many patients, that means Chronic Care Management. Some patients may also use remote monitoring or pharmacist support when it fits the care plan.
2. A care coordinator reaches out
A care coordinator may call on behalf of your doctor's office to explain the program, answer basic questions, and make sure you understand what support is being offered before anything begins.
3. You get support between visits
That support can include regular check-ins, care coordination, medication-related follow-up, and help keeping track of the next step in your care plan. If remote monitoring is part of your program, it can also include support around approved devices and the readings they send.
4. Your doctor stays informed
Your doctor remains the provider of record. Important follow-up, documentation, and monitoring information can be communicated back to the practice so your care team stays aware of what is happening between visits.
